Read that again, because it is the single most important sentence in this article. You will be paying staff, rent, insurance, and compliance costs while receiving nothing.

The licensing consultancies do not warn you about this because their job finishes at the licence. The gap is somebody else's problem, and that somebody is you.

Why the gap exists

Rates vary enormously by state, by waiver, and by service code, and they change. Every number below is a published range used to show you the arithmetic, not a promise about what you will be paid. Verify current rates with your state agency before you commit to anything.

For a assisted living facility, 3 to 6 months between licensure and your first Medicaid payment. That is not a delay caused by anybody doing anything wrong. It is simply how the enrolment and payment process works.

You cannot bill before you are enrolled. Enrolment cannot start until you are licensed. And the processing takes what it takes, regardless of how ready you are.

Meanwhile the property costs money from the day you sign. Staff cost money from the day you hire. Insurance costs money from the day you are covered.

What the gap actually costs

Work it out for yourself, because the number will change what you do.

Take your fixed monthly costs. Rent or mortgage. Insurance. Any staff you must have on the payroll to satisfy licensing. Utilities. Compliance.

Multiply that by the length of the gap for a assisted living facility. That figure is what you need in the bank before you open, on top of everything the startup guides told you about.

Most people budget for the startup costs and not for the gap. That is why they run out of money after they are licensed and before they are paid, which is the most frustrating possible way to fail.

You can do everything right, be fully licensed, have a beautiful facility and qualified staff, and still fail in the gap. That is not a failure of effort or of care. It is a failure of arithmetic.

Week two, separate the money completely if you have not. Business account, business only. That is what turns a licensed operation into an asset somebody could buy or lend against.

Week three, write down how the standard day runs. One page. That page is the beginning of a business that could exist without you standing in the middle of it.

Week four, request the DUNS number. It is free, it takes a few days, and the credit identity takes six to twelve months to build. Starting now means having it when you need it.

That is a month, it costs nothing, and it moves you further than most owners in this sector move in a year.

The numbers to know about your own assisted living facility

Very few owners in this sector can answer these quickly, and each one is worth an afternoon.

What does your operation actually net, after everything, per hour of your life? Including the nights on call. Including the shift you covered because somebody did not turn up. Including the compliance paperwork nobody bills for. That figure is the honest measure and almost nobody calculates it.

What is your occupancy across a year rather than today? A full house this month tells you very little. The pattern across twelve months tells you whether the business is stable.

What share of your revenue comes from one payer? If the answer is most of it, a rate change or a policy shift takes the whole business, and a lender or a buyer would spot that immediately.

Could somebody else run this from what is written down? If the honest answer is no, then everything the operation knows lives in your head, and a head is not an asset anybody can value.

The tedious thing that changes everything

None of this is exciting. Separating the accounts. Writing down how the standard day runs. Requesting a free identifier from a company most people have never heard of.

But that is precisely the work that converts a licensed operation which makes money into a licensed operation which has value, and the gap between those two is where most owners in this sector get stuck for years without realising there was a gap.

It also compounds. An operation with two years of clean records is in a completely different position from one with two years of good intentions, and the difference was an hour a month.

Start now rather than when you need it. Business credit takes six to twelve months to establish. Records cannot be created retroactively. The version of you who needs this in two years is depending on what you do this month.

An honest word about this sector

I want to be straight, because a great deal of what is written about this business is written by people selling licensing help.

This is a genuinely demanding business. The regulation is heavy, the staffing is difficult, the reimbursement is slow, and the responsibility is real. Anybody presenting it as easy money has not run one.

It is also a business where the arithmetic works for a lot of people, where the demand is not going away, and where a well-run operation becomes a genuinely valuable asset.

Both of those things are true. What decides which one you experience is almost entirely whether you did the arithmetic before you signed the lease, and whether you built the systems before you needed them.
